How to Submit a Model Release Form on BentBox
A quick 4-step guide for photographers and creators to attach a model release form when uploading content.
Are you the only performer, or does your content feature other performers / models?
When uploading content, BentBox will ask you to identify your role. "Performers" here means anyone — including models — who appears in your photos or videos.
If you are the sole performer in the content, select "I am the only performer in this content" — no additional release form is required.
If you are a photographer, or your content features other performers / models, select "Other performers appear in this content". You will then need to provide a release form for each performer / model.
How do you want to attach the release form?
Paste your existing ProntoID Form ID directly into the field in Step 1 on the upload page — "I already have a Form ID from BentID or ProntoID". This is the fastest and most reliable method.
If you need to submit a new paper release form, choose Option 2: "I need to create a new release form". You will be guided through a secure submission flow on ProntoID.
Complete the release form on ProntoID
You will be directed to the ProntoID integrated page, which runs on ProntoID's secure infrastructure. Fill in all the required fields for the performer / model.
Upload one file per upload area: one for the model release form and one for the performer / model's ID document. If either document consists of multiple pages or files, zip them first and upload the zip file.
Privacy tip: we recommend encouraging your performers / models to create their own verified ProntoID account. When they are verified on ProntoID, they manage their own identity — you never need to handle their ID directly. This is a significant privacy and compliance advantage for both parties.
Your release form has been processed — copy the Form ID
Once your documents have been securely uploaded and processed, ProntoID will generate a unique Form ID for that release form.
If you have a ProntoID account, this Form ID is permanently stored in your dashboard. For any future upload featuring the same performer / model, simply retrieve the ID — you will never need to re-upload their release form again.
Paste the Form ID back on BentBox to complete your upload
Return to the BentBox upload page where you started.
BentBox will instantly verify the Form ID and link the release form to your content. Your upload is now compliant and ready to be published.
That's it! Next time you upload content featuring the same performer / model, simply enter their existing Form ID from your ProntoID dashboard — the whole process takes just seconds.
